Outlook toolbar application that communicates with Web Service

I need two 'forms'. A 'Reminder Settings'when they create or modify an appointment, and a 'Reminder Preferences'. When someone creates a new appointment or modifies an existing one, I want to have an option called 'Reminder Settings'. If they select that option, a popup appears that allows them to enter: 1) Their web service e-mail (16 character string) 2) Their web service password (16 character string) 2) The appointment title (should be pre-populated) 3) The recipient they wish to send to (10 characters) 4) The appointment date & time (should be pre-populated) Both the web service password and recipient should be pre-populated if they have set those in their references page. Preferences page should be integrated within Outlook menu. Once the user hits 'Ok' on the 'Reminder Settings' form, the following are sent to the web service: string Record(string Email, string Password, string Appointment, string Number, string DateAndTimeOfAppointment) The web service returns a string in the form of "XXX - ResultString" where XXX is a three digit code. Please display a dialog box showing the return string once the user clicks 'Ok' from the 'Reminder Settings' form. The 'Reminder Preferences' form allows the user to input default values for e-mail address, password, and recipient. The web service host address will be hard-coded into the application and cannot be changed by the user. ## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):

a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).

Needs to work on Outlook 2000/2003 on Windows 2000 and Windows XP.
