Create Web Forms from Excel example...

I am looking for a coder who can do the following: 1) Create a web-form based on the 2 sheets in the attached excel file. They should be part of the same web form. 2) Setup the form you create to access our SQL database and create a new record and post the submitted data to the corresponding fields within the created record. 3) Put 2 buttons on the page. Submit and Clear. Once submit is clicked, the Clear button should change to Start New (still just clears the form and resets the buttons to the original actions), and the Submit button should become Go To Encompass which would invoke a program on the user's computer. 4) Somewhere we should also have a note that tells the user "Once you click Submit, Encompass must be used to edit the data entered. Do not click submit more than once for the same Borrower." ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform Will be running form on a Windows server.
