We are in the process of creating a small assesment (survey tool) for an application we are building. The primary database structure and associated pages are built, we are looking for a developer to complete the backend code for the following tasks:
1. Program data entry screen to add and edit new assessments and populate associated ‘Assesments’ table. Prefer one page to handle both functions ([login to view URL]).
2. Add new field upon selecting “Add?? from the questions section of the page.
3. Populate ‘AssesmentQuestions’ table with data entered by the user in the ‘question’ fields.
4. Populate ‘AssesmentModules?? table with ‘MenuID’ selected from the drop down menus (drop down menus populated from include page: [login to view URL]) and add new entry point to allow for additional menuid’s to be associated with the question entered in #3. Note: Multiple MenuID’s need to be associated with each question, hence the reason for the AssesmentModules table versus adding a MenuID to the AssesmentQuestion table.
5. Utilizing the same page as the add process ([login to view URL]), create a process to modify existing questions and/or add new questions and associated menuid’s as described in steps #2 and #3 above.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
