ASP.NET Developer for EHR (for temlate library)(repost)

We are looking for a developer to add some additional functionality to our web based EHR application. The new functionality includes a new template library. This library allows the user to manage (add/edit/delete) their forms (eg. intake forms, etc.). These templates are typically in the form of HTML documents, that then dynamically generate a user form that allows the users to enter data. The technologies include ASP.NET, VB.NET, SQL Server. This is a stand alone piece that will need to be encorporated into our existing client facing application. ## Deliverables please see attached
