database filter

write down a PHP code which is going to filter some data from MySQL database. This script must consist of 2 parts: user interface and data entry part. simple idea of script is the following: user enter it's parameters, script filter all the information in database and shows in table the results what that script must do in details. (please see attachment named image1 ) 1) filter parameters where user can choose or enter 2) ability to enter information to database from special page (the page with password) 3) script must be in Ajax and PHP 4) beautiful design.
