AJAX, php and MySQL data webpage display

I need a complete webpage that would read my data from mysql database then display on the web server (web page). This webpage must be a php ajax program/webpage so it will look and feel like a windows application. The webpage will consists of several sub pages including 1. Login page 2. Display page (Action page) 3. Information page 4. Setting page Login page Before users can gain access to page 2-4, he/she needs to login to the system first. I already setup a user table for you in my database. This table contains user name/password, time allow for log into the system and privilege. A given use can login to the system within his/her time allows or the system will reject his login and an email is send to the system administrator. If lower privilege user login to the system, he/she may be able to have access to the action or any predetermine page (set in setting) only. Display page (Action page) This page will display four columns in thumbnail picture, tag ID, first/last name and tag read time/date (call this a card). The text colors for tag ID and time/date will fade out at a predetermine time (set in the setting page). For example, if the timer is set to 15 seconds, and there are 5 tags in the database, as the page read these data, it will display 5 pictures along with tag ID, names and tag read time/date in let’s say red. Five seconds later, the text color will fade out every 5 second for 4 times. I would also need a “display all”, “filter” and “print” button on top of this page. If a “display all” button is press, all thumbnails read from beginning of that day will be displayed, other wise, “filter” button is used for displaying a certain periods of the day. Information page The information page will need to look and feed like a searching program. This page will consist of user information (i.e. user tag ID, picture, first/last name, phone number, address, emergency contact, etc). The page will have an “auto fill/search” function, where the user will input user info in any field and the rest of the user info will auto appear. For example, if I put user ID in the ID box, user name and everything else will appear automatically. This page will be format pretty much like an application form, I will supply a sample of the page and what it would look like to you. Setting page The setting page will consist of all settings that is needed in order for the program/page to function properly. This may include the database address and login info, time for text fade out, number of color fading before the user picture; tag ID and read time/date disappear, in-active time out, page display for lower privilege user, etc. I will provide you with a test server upon your delivery the final code so it can be tested on my server. Until then, you will work on your own server but I will supply all data tables that will be in my database. This way you can see how your program is going to talk to my database. You will need to work with me closely as I would like to approve what the program will look, feel and functions on every step of the way. This way there is not confusion that would delay our final delivery date (I’m online most of the time). What I would at the end of the project is the final functioning source code, working code tested on my server, and nothing else. Upon delivery, I will need several days to test the page stability and functions. This program maybe extremely simple for someone who is proficient in ajax and php so I would imagine this would not very long time. The only thing need to be concern is the look. I would like to have this pages look profession, easy to use and stable.
