Create PDF: Exchange of Information

I need to take some forms and re-create them as PDF forms in two formats. One format will be an unlocked version that I can access and make changes to as my business needs change or grow. The other format will be a locked version of the form that can be emailed to clients to that they can fill in but cannot change the form itself. Exchange of information form Counseling Claremont: one version should be for Counseling Claremont. This form should be identical to the form being uploaded here. Exchange of information form Bright Road Recovery: second version of this form should be identical to the first, except for: 1) contact info at the top should read Bright Road Recovery/428 Harrison Ave #101/Claremont, CA 91711/Phone 909-994-1436 Fax 909-624-7002; 2) color should be different than Claremont Counseling form; 3) First line of text in the body of the form changed to: I, the above named client, authorize Bright Road Recovery (and its staff) and the following providers and/or individuals to exchange information about my treatment.
