Apple + Android Simple App

I would like to have a simple application developed to integrate with my ticketing system API. I want a simple mobile app for both platforms to have a logo and three buttons. First button is to create a new service request. Second button is to check on an existing service request. Third, a button to call a phone number from the phone.
