Customer Scheduling App for Service Company

I am looking for a developer to design an app in iOS and possibly Android as well that can allow our customers to schedule a service call or make an appointment. This information will need to be sent to our office in some format that is readable and storable (A client/server system could work where our office has an app that keeps record of this). I would like customers to be able to create their own accounts so that they could log on and would not have to fill in their address and information each time. The only options I want them to have are;
1) A dropdown list of common problems including one option to type a new option,
2) The degree of the emergency,
3) Whether they want someone out right away or if they would like to schedule for later in the day, night, or next day,
4) If overtime is approved
5) Ability to add new customer
6) Customers will need to store their location information and other data in their account profile that will be sent along with their schedule request
7) Confirmation needs to be sent to customer along with a signature page that they can sign once service is complete (I would also like to have a rating system on this where they are required to rate the service technician and overall service as well as leave comments. Ideally the ratings would be tied to each employee and a history of ratings would be acculated
Customer App Side: On the app side, I need this to be available for free to the general public through app stores with no advertising. I have an apple developer ID and would like to distribute through that. I need this app to allow customers to register with required information such as store name, address, phone number, email, fax, contact info, payment information (credit card), and possibly other optional settings. Once registered, I want customers to be able to place a service request through the app, automatically sending their information to our server side. When scheduling the service request, they will need to be given options including;
1) A dropdown list of common problems including one option to type a new option,
2) The degree of the emergency,
3) Whether they want someone out right away or if they would like to schedule for later in the day, night, or next day,
4) If overtime is approved
Once we receive and schedule the service call, I would like confirmation to be pushed back to the customers app. After service is complete, I would like an option for the customer to sign on the screen and rate the service technician and overall satisfaction. I would like this information to be able to be stored on our service side, aggregating average rankings for each service technician. I would also like customers to be able to access history of their past service call schedules and access them from within their app.
Server Side: On the server side, I would like to have the ability to give multiple people access in our office to receive a push notification when a service call is requested. Since multiple users will be able to see this, I would like to see it with an "open" status until one user claims it, giving it a "in progress" status, and then once the user that claimed it successfully schedules it, it will be given a "closed" status. As mentioned above, I would also like to keep customer rating data and past customer service call information on the server side. I will like to see the average rating that service technicians receive and be able to access old service orders.
Employee App Side: For this side, I would like this to be a separate app that is not available to the public and can only be distributed to our employees for free. (This only needs to be an iPhone app). I also need this app to communicate with the server side. Once the server side receives a request from the customer app, the server side can schedule the call with an available service technician and send the order to the service technician through this app. Employee will receive push notification on this app of scheduled service order with customer information as filled out by customer on the Customer App as described above. The technician will need to add items such as his time spent on the job, inventory and materials used, and two sets of notes (One set that the customer can see and one set that can only be seen on the server side). Once service order is completed, technician will have to get a signature from the customer and the service order will be sent back to the server side in full completion and to the customer app with only a receipt. If customer does not have the customer app, it will need to be sent to their email address. (This whole system needs to work even if the customer doesnt have the customer app)
An additional feature that I would be willing to increase the budget for would be to allow this to integrate with our Quickbooks company file. Quickbooks is an accounting software that we use and store all of our customer information in. I would like the service order to be able to be transferred directly from the server side into our Quickbooks software, automatically creating an invoice as well as filling out a timecard in quickbooks.
Also, I would like to have ownership of this entire programming so that I can distribute it at will on my own later on.
