Convert PSD (simple designed pages) to Responsive HTML

I am building an app with a very minimal design that needs to be responsive and follows the same basic template. I would like it to be made with bootstap, but if it works cross-browser and is responsive, it is not 100% necessary. I have attached some screenshots of the PSD. There are about 17 pages total, but most of them are just forms, and I've already coded a basic template with the header and body. Included zip file of template page. I would like the nav to collapse like bootstrap does, for smaller viewports. Also include field validation for forms.
