Development of question set for Web Application user survey

We are looking for someone to develop a set of questions for a survey based on new/existing users on a web-based application. The application itself is not important as the questions should be geared towards any application of this type. The questions should cover areas such as; Design Usability Help Features Speed and whatever else you feel can be added. I would like to survey to contain between 10 and 15 questions, although I am open to suggestions from experienced people in this area. I will be happy to answer any questions.
