Children's Book Illustrator and Layout/Formatting for Kindle/CreateSpace

I have a children’s book that I wrote and I would like to see it published via Amazon Kindle and CreateSpace. (Ideally both digital and hard copy formats). I am looking for an illustrator to bring my story to life. (Illustration and text formatting/layout). Basically, there are 23 four line verses. I envision each verse would appear on one side of the page (right or left) with some simple sketches in the background like in the corners or along the bottom of the page. The other side of the page would be a full illustration. In addition, I will need a front and back cover (the front cover could be one of the pages from inside the book), a page with copyright information, a page for dedications, and an About the Author page. If there is anything else you think I would need, I am open to suggestions. I am also open to what materials are used to draw the pictures such as water colours, etc. The final document needs to be formatted for Kindle and CreateSpace so you must know how to do this so that I can simply upload the finished document. I am looking to pay a fixed one-off fee. While I regrettably can’t share any royalties with you, I am more than happy to give you credit as the illustrator and even include a paragraph about you on the About the Author Page. Before selecting an illustrator, I will contact any bidders that I am interested in and ask for a sample drawing of the main character to see your style and how you can bring the character to life.
