Data-gathering poll with live graphical output that can be integrated into an interactive HTML infographic

I am currently building an infographic which needs a data-gathering poll which will feed out current poll results in a graphic display. Users will be asked a simple yes or no question, the user will then be returned a graphic showing current poll results. This needs to be implemented into a HTML (with some HTML5 component) page. The server will be capable of PHP, HTML and can use an SQL database if needed although we would prefer not to implement this.
