Create optin pages, integrate autoresponder and build pages with forms

Find the attached image. 1. I want autoresponder integrated to the first (1) page ie. when they enter their email id and the PP id they will get subscribed to my autoresponder immediately. 2. Once they fill the first form and click on next they will be redirected to second page (2) 3. Once they fill the second form and click on submit they will be redirected to third page. There might be some more additional minor changes that can be required.
