Citation Generator

I basically need something like this completed:[login to view URL] It would also need to utilize the most recent versions of MLA and APA MLA - latest version = 8th edition [login to view URL] APA - latest version = 6th edition [login to view URL] I want it to be much smaller then their design, nicely designed, and in html form so I can easily add it to any html website and it will work correctly for the user. the design and colors should be generic so they match any website. Basically user fills out information on the book, newspaper, webpage, journal , web image, online video, magazine, blog. User would select from drop down the kind of source (listed above), and the citation method (APA 6th Edition, MLA 8th Edition, Chicago, based on their choices other fields would populate, they would fill out that information (some fields would be required), then once they click "Generate" the script would generate them the correct format for the bibliography or work cited page which they can copy/paste. It would also need some type of captcha so robots can't abuse it. I attached image of how competitors form looks and want it to function similiar but look very different, cleaner, etc.
