Database to CSV file for multiple clients and basic white label CRM might include some graphic design and coding if capable

Our Kiosks are mostly used for collecting contest entry data. I need a few tasks completed on an ongoing basis for each client - Graphic design for Kiosk as attached - Landing page for contest data entry - Data to CSV file with easy access as needed by client - Looking for white label CRM for clients - 1 graphic panel created ASAP. - Looking for kiosk app for locking pages and motion activated app for voice prompt as well. This might fall into a one time programming job.
