I need a PDF form created in Adobe LiveCycle Designer which will be read by a ColdFusion 9 server CFML application. The form has about 20 sections, 3-5 columns each @ 4-8 rows. Each column must have unique field-name assigned. Each section needs to be created as a group. The form, when printed, is about 6-8 pages. Previous experience with this sort of work is required. All source must be provided when accepted. Please provide examples/description of work similar if possible.
