CD Jewel case inserts- produce eps and pdf files from design in Word

I have some designs for CD Jewel case inserts. I am happy with the designs. They have been created in MS Word which is not a very suitable software and I now want the designs moved to Illistrator or similar and changes to make them easily printable plus creating eps and pdf files. I want the eps files so that they can be used for making future modifications or as templates for the next set of CDs. I am open to your reccomendation of other options or better approaches if you think what I am asking for could be better delivered in another way.
General requirements
* All copyright and rights to use the files, designs and works for any purpose and without restriction belong to Lynnette Dickinson and the freelancer/designer retains no rights over the works produced.
* Before final payment draft copies must be provided that can be printed and trimmed to check that they fit the jewel cases. Final payment to be made when design and correct fit have been confirmed.

What I have now for the back folded insert:
I have an acceptable design that has been created in Microspft word for three CD Jewel case (Back) inserts. The A4 sheet has one insert on half the sheet for each of the current designs.
Each design follows the same template with modifications for the specific text of each CD. IE There is a single "template" design and 3 specific CD inserts using the same basic design.
I have atatched the files.
Deliverables for each CD (3 of) for the Jewel case back inserts.
* A vector EPS file
* A PDF with bleed and trim lines
* A PDF with no bleed, where the image is exactly the right size if the white space is timmed to the edge of the image
* Each file must fit two inserts onto one A4 page
* Page size is A4 for all
* Image sizes and trim lines are must be correct for Jewel case inserts, so that there is a neat fit when correctly trimmed.
What I have now for the folded front insert
I have a word document that has a number of lines that if trimmed to just remove the lines have it a neat fit to the CD case. One document for each of 3 CDs.

For the current design -
* A vector EPS file
* A PDF with bleed and trim lines
* A PDF with no bleed, where the image is exactly the right size if the white space is timmed to the edge of the image
* There will be one insert per A4 page
* Page size is A4 for all
* Image sizes and trim lines are must be correct for Jewel case inserts, so that there is a neat fit when correctly trimmed.
A modified design -
* The CD name and author is added to the front (image) part of the insert with a design similar to the CD face design. CD design files are attached for reference.
* This could be as simple as just adding the required text in top of the image in a layout that is acceptable to me. (Same font as the CD fronts)
* You may offer another design idea for the the jewel inserts and the CD fronts, but it does need to work with the CD backs. [The idea is to simply add CD name and artist name to the front of the CD inserts, but we would consider other ideas if they looked really good to us]
* Once the design is agreed on then the deliverables would be
* A vector EPS file
* A PDF with bleed and trim lines
* A PDF with no bleed, where the image is exactly the right size if the white space is timmed to the edge of the image
* There will be one insert per A4 page

We are hoping that this is a pretty quick job that is more about creating the required files rather than creative design work.
