Creating a custom Google Map with 30 locations pinned

Basic project for someone who knows how to work in Google Map Engine - My Maps

I need a map similar to the link below.

I will provide approx 30 addresses (soccer fields) and they need to be posted on a google map that I can share.

I'd want a custom legend as the one below with icons as the example below, vs the standard google map pins.

I'll have more of these in the near future that need to be done.
