Database/Spreadsheet Creation

I need a database/spreadsheet created where the following data is tracked: (SEE ATTACHED FILE FOR FORMATTING SUGGESTIONS) Client Number; Client Name; Date; Task Completed; Number of hours worked on task; Total number of hours worked during the month for specific client (running total); Total numbers of hours worked during the month for all clients running total); Total number of hours worked during the year for specific client (running total); and Total numbers of hours worked during the year for the specific client. When you enter the Client Name, the Client Number should automatically pop up (pre-fill). The date should be searchable by Client No., Client Name or date.
