Data Entry Required - Contest Forms

We require to have several different contest forms completed and submitted on our behalf. There will be approximately 6-9 different contest forms that will be used. We require approximately 3,000 - 5,000 forms completed for each contest. Here's an example of one of the contest forms that we will be using: [login to view URL]
In total we will require 20,000 - 30,000 entries completed.
All data will be supplied for you (including Names, Addresses, Email Addresses, and Phone Numbers). Correctly entering the VALIDATION KEY is also required.
We require to have the IP Address reset / changed & cookies / cache cleared after every 10 applications correctly submitted.
