Create editable checking account statement

Need editable chase checking account statement template created. Needs to be very user friendly and able to access in average excel program. Template needs to be exact replica of original as regards to layout, design, and features including bar code. The template must be fully editable to allow changes to all information on statement including bar. Copy of statement to be provided to selected person. Need asap
