ASP.net add field to form and to [login to view URL]

I have a webform on a asp.net site that has some fields that are integrated with [login to view URL] and we need to add 1 more field.

you should be familiar with [login to view URL] or be able to quickly figure out how to add 1 more field to the system. Our webform seems pretty simple so you will just be adding a new check box that will be next to others that are already working and integrated.

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

(No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
