Create web forms in asp classic, capture data to ms access

I need 9 forms creating in asp classic. These forms need to capture data and send the results via email to a particualr email address. The results also need to be captured to an ms Access database. There are a number of required fields on each form that must vailidate. The email should be created/sent with CDO. Once the data is emailed/captured, you will redirect to a thank you page.
I will provide .asp template pages for each of the 9 forms, and the thank you page. You will need to create the form input fields based on the questions that are in an excel spreadsheet. You will also need to create a seperate table in the access database to capture the data from each of the 9 types of form (so 9 seperate tables in total). I will provide the winning bidder with ftp access to the webspace where the forms will live. I have also created an asp connection string already that will connect to the live database when it is uploaded.
I have attached question sets so you can get an idea of the amount of work involved.
IMPORTANT: This must be completed in ASP Classic NOT .NET.
