im a freelancer specialized in designing , i want multiple surveys or forms to be written in order to determine my clients needs and requirements so i can start designing their projects right after they submit the forms.

subjects that i need to be covered with surveys / design request forms :

-3D rendering.
-Architectural design: (residential-commercial-educational-healthcare-retail-offices).
-Space planning: (residential-commercial-educational-healthcare-retail-offices).
-Landscape design: (back yard-front yard-farms-parks).

**the forms are to be written in word format .
**priorities will be given to architectural designers / engineers.
**this task needs a person who's aware of designers mentalities.
