Contest Brief

We have 2 web pages that we use to collect customer reviews. I am looking for a talented web developer/designer to give the pages a much needed visual (UI) redesign!

The design should be modern, clean, functional and mobile responsive.

JS/CSS frameworks may be used but pages must be quick to load.

Original HTML files and screenshots attached.

The main company logo will be changed so don't base a color scheme or design on this logo.

******** PLEASE NOTE ***********
1) I need to see designs for BOTH pages.
2) DO NOT submit an entry without a design.
3) A lot of the entries being submitted are too similar to the original pages! The brief is to give the pages a visual (UI) redesign that improves the look of the pages. Please experiment with layout and design!
