Contest Brief

This website is for a pet crematory, where the pet owner goes and chooses a cremation service package for their pets.

The packages catalog is very limited (about 10 packages) so there wont be search/tagging/etc involved, it is just straight service catalog selection.

The website consists of 5 screens:
1. Owner and Pet Information capture
2. Pet weight and Package selection
3. Authorization release (text authorizing their pets remains to be cremated that the owner must accept thick a checkbox)
4. Order summary and payment
5. Pet owner dashboard
