Contest Brief

I need graphic design developed for a contact form that will appear on several pages of the site. An example can be seen in the blank section on the right hand side of these pages:

The form needs to use the attached code and should fit within the width of blank section specified above. A new button needs to be developed for the form as well. The button should say, "Free Consultation", "Get Started Now", or "I want a better website", etc. Basically, anything other than "Submit".

The design of the form should match the color scheme and feel of the rest of the site while still standing out enough to grab the visitor's attention. There should be a header on the top of the form saying:

"Only for Serious Business Owners
Get Your Free Consultation Today!"

The next line should say:
"Let us help make your website into the business development tool it was always meant to be. Fill in the form below to get started."

All graphics need to be made using either Adobe Photoshop or Adobe Illustrator. If your design is chosen, the unflatten graphics files will need to be provided upon completion of the contest.

This should be a fairly easy project.

Hello Everyone - Thank you for your entries. I will be contacting each of you to discuss your designs over the next couple of days. In the meantime, please keep in mind that the project is for graphic design of the form. I am able to add words to the form myself so I'm looking for someone who can make the form look great. Simply adding words to the top of the form does not count as graphic design.

One thing I failed to mention is that this form will show up on other pages of the site so the color scheme should be based on the major colors of the site and just the colors of the particular page. I will edit the project brief to clarify. Thanks.
