I will send you a word document with all the information and wording. All you need to do is format it, so i can send to clients and they can easily fill in the form. I will send an example also. I need someone with good computer skills. Please make sure that you do not send a complicated pdf that is difficult to fill out.

Please use attached logo. Perhaps change colour of writing or use black background so it stands out
